Seems like it's about that song that Chester was singing on the Making of Meteora DVD. It was used in the background of an LPTV episode too, but that's all we know about it. It's in the full version of the Summer Sanitarium LPTV episode.

The Smallville episode is "Crisis" from the 3rd season. I have it on DVD. I remember that episode because Clark was using a green Alienware computer

It's funny, I pretty much hate both of those songs in terms of LP (but I like the guitar and bass stuff in the verses of Forgotten), but I LOVE the Reanimation versions.
